We’d like to thank you for considering The Montfort Group for your counseling needs. Your appointment is confirmed! The Montfort Group specializes in helping our clients maximize the quality of their lives and relationships through counseling. We sincerely hope that you can feel that from our first conversation together. 

Is this appointment for couples counseling?

If you will be attending with your spouse or partner, we will require completed documents for you both. Please either provide their name, email and phone number or have your spouse or partner contact us, referencing your appointment about being added to the system so we can set up their own account. 

Are you booking for someone under 18?

For the initial assessment for adolescents/minors, we ask that only parents attend so we can gather proper background information and treatment goals from the parent perspective. The second session will be the client without the parent present. We do not accept clients under 13 but are happy to provide a referral. Please provide any legal documentation required for consent to mental health treatment.

Intake instructions

You should have received an email from us with a link to set up your secure online Client Portals. If you do not see the email then please check your junk/spam folders. Please email us if you cannot find this email.  We do require that all intake documents and questionnaires are completed before your session and that a valid payment method has been added to your account. 

Your online portal

Once you’ve set up your username and password, click the Documents tab and just make sure you complete all intake forms before your session. You can be as detailed as you like in the questionnaires; they will only be seen by you and your therapist.  

IMPORTANT: Whenever scheduling online, please be aware that you will need to choose the clinician of your choice from the drop-down list in order to ensure you are looking at the correct availability with the correct therapist. 

Appointment reminders

Please also be aware that text and email appointment reminders are an automatic courtesy service through our software; any text responses will not be seen by TMG staff or therapists. If you need to cancel or reschedule your appointment, please make sure to let us know 48-hours in advance via voicemail and email so as not to be charged. You can view our full cancellation policy in the Documents tab and here on our website.

Payment information

We do not accept insurance but are happy to provide the coding necessary for you to go through reimbursement on your own. Since we are out-of-network, regarding reimbursement from your insurance company, we recommend contacting them via phone, telling them you are planning to see an out-of-network therapist for mental health treatment and would like to know the reimbursement rate according to your plan and what the steps would be for filing for reimbursement.
